Valid and signed U. S. Passport.
One Visa Application Form Must be compeleted online
Two (2) passport size photograph
Letter of invitation from sponsor in Nigeria,If applicant’s sponsor is Nigerian citizen, provide a copy of sponsor’s Nigerian passport.
Copy of travel itinerary (Optional )

Service Order Form
Non US Citizen Must Provide a clear copy of Green Card or Valid Legal Status of H1-B, H-4, L1 etc

Ship Your Documents:

You must call Travel Monitors prior to sending your documents if requesting a 24hrs or Same Day service.

Same Day and 24 hour request must be received in our office by 8:00 AM you must select NO SIGNATURE REQUIRED

Document received in our office after 9:00 AM Will be submitted on the next business day
