Service Order Form
*Non US Citizen Must Provide a clear copy of Green Card or Valid Legal Status of H1-B, H-4, L1 etc

BUSINESS VISA

Valid and signed U.S. Passport
One Visa Application Form (must be completed online)
Two recent passport size photographs
Applicant’s Business Letter from the employer or sponsoring company on company letterhead
Copy of travel itinerary (optional)

8:00am -2:30pm (Monday – Friday) by Appointment till 4:30pm9:00am -2:00pm (Saturday)
12:00p.m – 4:00pm (Sunday) By appointment only

Ship Your Documents:

You must call Travel Monitors prior to sending your documents if requesting a 24hrs or Same Day service.

Same Day and 24 hour request must be received in our office by 8:00 AM you must select NO SIGNATURE REQUIRED

Document received in our office after 9:00 AM Will be submitted on the next business day
