Special Visa Requirements For Minor Under 18:

• Both parents must sign the Visa Application Form A
• One passport size photograph
• A copy of the minor birth certificate
• Both parents must provide a clear copy of drivers license

STUDENT VISA

• Valid and signed U.S. Passport
• One Visa Application Form A
• One Visa Application Form B (Supplementary)
• One (1) passport type photograph
• Send a copy of your approved Foreign Student Form (JW101 or JW202)

BUSINESS VISA

• Valid and signed U.S. Passport
• One Visa Application Form A
• One(1) recent passport size photograph
• Applicant’s original Business letter on company letterhead ( optional ).
• An Invitation Letter ( fax or Scan copy ) issued by companies, corporations, institutions and individuals in China.
